FIRST TIME/NEW PASSPORT FOR APPLICANTS AGE 16 AND OVER

Eligibility:

• Applying for a U.S. passport for the first time; OR
• Have a ordinary U.S. passport that was valid for FIVE YEARS; OR
• Have a U.S. passport that was issued MORE than 15 years ago; OR
• Have a U.S. passport that was issued when applicant was under age 16.

You must bring the documents listed below and appear in person at an Acceptance Agent. They will witness and seal them in an envelope.

1. Application Form for U.S. passport (DS-11). DO NOT SIGN FORM DS-11 UNTIL ACCEPTANCE AGENT ASKS YOU TO. Please note that application must be presented to the U.S. Department of State within 5 business days of the date on the application.

2. ORIGINAL proof of U.S. citizenship. (See Execution Instructions for examples.)

3. Two identical color photographs, 2" x 2" in size, with a plain white background and taken less than six months ago.

4. Itinerary is required for processing time less than two weeks. Fail to do so will significantly delay the process.

5. Check or money order payable to "U.S. Department of State". Please note that starting check will not be accepted. See Government Fee below.

Attach the following outside the sealed envelope:

1. Two Letters of Authorization. These are form letters authorizing our company to process your passport request.

2. Passport Processing Order Form. This will allow us to see the name of the applicant, along with the date of birth and date of departure.

3. If you are also applying for visas(s) through us, all the requirements for visa(s) must be left outside the sealed envelope.

Government Fee (to be sealed in envelope by Acceptance Agent):

• Passport Book: $170.00
• Passport Card only: $90.00
• Passport Book and Passport Card: $200.00

Service Fee (to be left outside the sealed envelope):

• Passport Book
  $45.00 - 15 business day processing
  $85.00 - 10 business day processing
  $125.00 - 5 business day processing
  $175.00 - 24 hour processing*

  * Reservation is required for 24 hour processing. You are required to fax us a Passport Processing Order Form with credit card information. Application must be sent via FedEx First Overnight or UPS Early AM Delivery, and must be marked "Signature Released" on package. Application that is not received at our office by 8:00am on the date reserved is subject to a charge of the full service fee.

  $45.00 - Regular processing

Return FedEx Express (to be left outside the sealed envelope):

$23.00 per address (up to three passports)

$28.00 per address (four to six passports)

$35.00 per address (Alaska, Hawaii and Puerto Rico, or Saturday Delivery)

You will need to pay $25.00 execution fee to Acceptance Agent.
