1. Your current U.S. passport. (Passport MUST be signed.)

2. Application for Additional Visa Pages (DS-4085), fully completed and signed by applicant.

3. TWO Letter of Authorization. These are form letters authorizing our company to process your passport request.

For the processing time of less than two weeks, you will need to provide an itinerary or copy of your air tickets that reflect a destination outside of the U.S. If you are requesting same or next day processing, you must show a departure date within 5 business days from date of your application. (Please contact us if you are requesting a passport amendment to be processed same or next day for complete details.)

5. Passport Processing Order Form completely.
