ADDING VISA PAGES TO EXISTING PASSPORT

Eligibility:

• Your existing passport is not damaged or mutilated.

REQUIREMENTS:

1. Your actual U.S. passport. (Passport MUST be signed.)

2. Application for Additional Visa Pages (DS-4085), fully completed and signed by applicant.

3. Two Letters of Authorization. These are form letters authorizing our company to process your passport request.

4. For processing time of less than two weeks, you will need to provide an itinerary or copy of your air tickets that reflect a destination outside of the U.S.

5. Passport Processing Order Form.

   * Reservation is required for 24 hour processing. You are required to fax us a Passport Processing Order Form with credit card information. Application must be sent via FedEx First Overnight or UPS Early AM Delivery, and must be marked "Signature Released" on package. Application that is not received at our office by 8:00am on the date reserved is subject to a charge of the full service fee.

   Return FedEx Express:
   $23.00 per address (up to three passports)
   $28.00 per address (four to six passports)
   $35.00 per address (Alaska, Hawaii and Puerto Rico, or Saturday Delivery)
