CHANGING NAME ON EXISTING PASSPORT


You may change your name on your existing passport due to marriage or court order.

For passport that was issued within one year, use U.S. Passport Re-application Form (DS-5504).
For passport that was issued over a year ago, use Application for U.S. Passport by Mail (DS-82).

To apply for name change on existing passport, please mail the following requirements to:

REQUIREMENTS:

  1. Your original valid U.S. passport. (Passport MUST be signed.)

  2. U.S. Passport Re-application Form (DS-5504) if passport was issued within one year;
    OR
    Application for U.S. Passport by Mail (DS-82) if passport was issued over a year ago.

  3. Two identical color photographs, 2" x 2" in size, with a plain white background. Photos must be of good quality.

  4. Two Letters of Authorization. These are form letters authorizing our company to process your passport request.

  5. For the processing time of less than two weeks, you will need to provide an itinerary or copy of your air tickets that reflect a destination outside of the U.S.

  6. Original document reflecting the new name in which you would like your passport to read, i.e. Marriage certificate, divorce decree, adoption papers, etc. The U.S. State Department will return your original document along with the new passport.

  7. Passport Processing Order Form.

  8. Fees.
