NAME CHANGE ON EXISTING PASSPORT

Eligibility:
  • Your existing passport is not damaged or mutilated; AND
  • Can provide original document reflects the name change.

REQUIREMENTS:

  1. Your current or most recent passport. (Passport MUST be signed.)

  2. U.S. Passport Re-application Form (DS-5504) if passport was issued within one year
    OR
    Application for U.S. Passport by Mail (DS-82) if passport was issued over a year ago.

    Please note that application must be presented to the U.S. Department of State within 5 business days of the date on the application.

  3. Two Letters of Authorization. These are form letters authorizing our company to process your passport request.

  4. Passport Processing Order Form.

  5. Two identical color photographs, 2" x 2" in size, with a plain white background and taken less than six months ago.

  6. Original document reflecting the new name in which you would like your passport to read, i.e. Marriage certificate, divorce decree, adoption papers, etc. The U.S. State Department will return your original document along with the new passport.

  7. Itinerary is required for processing time less than two weeks. Fail to do so will significantly delay the process.

    • Passport Book: $170.00
    • Passport Card only: $90.00
    • Passport Book and Passport Card: $200.00

    Service Fee:
    • Passport Book
      $45.00 - 15 business day processing
      $85.00 - 10 business day processing
      $125.00 - 5 business day processing
      $175.00 - 24 hour processing*

      * Reservation is required for 24 hour processing. You are required to fax us a Passport Processing Order Form with credit card information. Application must be sent via FedEx First Overnight or UPS Early AM Delivery, and must be marked "Signature Released" on package. Application that is not received at our office by 8:00am on the date reserved is subject to a charge of the full service fee.

      $45.00 - Regular processing

    Return FedEx Express:
    $23.00 per address (up to three passports)
    $28.00 per address (four to six passports)
    $35.00 per address (Alaska, Hawaii and Puerto Rico, or Saturday Delivery)
