REPLACING LOST, STOLEN OR DAMAGED PASSPORT

You may use Application for U.S. Passport (DS-11) if you:

Please have the following documents witnessed and sealed by an Acceptance Agent:

  1. Application Form for U.S. passport (DS-11) must be completed and printed out from the U.S. Department of State web site, Application for U.S. Passport. Failure to do so will significantly delay your application. After entering the information, click "Create Form" and print out the completed application Form. DO NOT SIGN FORM DS-11 UNTIL ACCEPTANCE AGENT ASKS YOU TO.

  2. ORIGINAL proof of U.S. citizenship. (See Execution Instructions for examples.)

  3. Statement Regarding Lost, Stolen or Damaged Passport (DS-64), fully completed and signed. This form is needed to prove to the U.S. State Department that you have lost or had your passport stolen. You must go into great detail on this form, stating where and when your passport was lost or stolen, and that you have exhausted every effort to find it. You must fully satisfy the agent that you are sure this passport will never surface again. In some cases, the U.S. Passport Office will issue a passport that is valid for only 1 year in hopes of you finding your lost passport. If the agent is fully satisfied, a 10 year book will be issued. In cases where your passport has become damaged or mutilated, you will still have to fill out Form DS-64, but include an explanation of how or why your passport was damaged.

  4. Two identical color photographs, 2" x 2" in size, with a plain white background and taken less than six months ago.

  5. For the processing time of less than two weeks, you will need to provide an itinerary or copy of your air tickets that reflect a destination outside of the U.S. If you are requesting same or next day processing, you must show a departure date within 5 business days from date of your application. (Please contact us if you are requesting your passport same or next day for complete details.)

  6. $135.00 in check or money order payable to "U.S. Department of State". Please note that starting check will not be accepted.

Attach the following OUTSIDE the sealed envelope:

  1. TWO Letters of Authorization. These are form letters authorizing our company to process your passport request.

  2. Passport Processing Order Form

  3. If you are also applying for visas(s) through us, all the requirements for visa(s) must be left outside the sealed envelope.

This will allow us to see the name of the applicant inside, along with the date of birth and date of departure.
