If you are applying for a first time passport or a renewal for a minor under the age of 16, you must follow the instructions below. The applicant, and BOTH parents or legal guardian must bring the following documents to an Acceptance Agent and have them witnessed and sealed.

Please have the following documents witnessed and sealed by an Acceptance Agent:

1. Application Form for U.S. passport (DS-11) must be completed and printed out from the U.S. Department of State web site, Application for U.S. Passport. Failure to do so will significantly delay your application. After entering the information, click "Create Form" and print out the completed application Form. DO NOT SIGN THIS FORM DS-11 UNTIL ACCEPTANCE AGENT ASKS YOU TO.

2. ORIGINAL proof of the minor's U.S. citizenship. (See Execution Instructions for examples.)

3. If you are renewing a passport for a minor, the minor's original birth certificate must be provided and sealed.

4. Two identical color photographs of the minor, 2" x 2" in size, with a plain white background and taken less than three months ago.

5. For the processing time of less than two weeks, you will need to provide an itinerary or copy of your air tickets that reflect a destination outside of the U.S. If you are requesting same or next day processing, you must show a departure date within 5 business days from date of your application. (Please contact us if you are requesting the minor's passport to be processed same or next day for complete details.)

6. If the second parent or legal guardian is not available to sign, he/she will need to fill out a Statement of Consent: Issuance of a Passport to a Minor Under Age 14 (DS-3053). This form must be NORTARIZED and should be submitted along with DS-11.

7. $120.00 in check or money order payable to "U.S. Department of State". Please note that starting check will not be accepted.

Attach the following OUTSIDE the sealed envelope:

1. TWO Letters of Authorization. These are form letters authorizing our company to process your passport request.

2. Passport Processing Order Form

3. If you are also applying for visas(s) through us, all the requirements for visa(s) must be left outside the sealed envelope.

This will allow us to see the name of the applicant inside, along with the date of birth and date of departure.
