If you are applying for a first time passport or a renewal for a minor under age 16, you must follow the instructions below. NOTE: Minors must appear in person with BOTH parents or legal guardians.
The applicant, and BOTH parents or legal guardians must bring the following documents to an Acceptance Agent and have them witnessed and sealed:

Application Form for U.S. passport (DS-11) must be completed and printed out from the U.S. Department of State web site, Application for U.S. Passport. Failure to do so will significantly delay your application. After entering the information, click "Create Form" and print out the completed application Form. DO NOT SIGN FORM DS-11 UNTIL ACCEPTANCE AGENT ASKS YOU TO.

ORIGINAL proof of U.S. citizenship. (See Execution Instructions for examples.)
If you are renewing a passport for minor, the minor's original birth certificate must be provided and sealed.

Two identical color photographs, 2" x 2" in size, with a plain white background and taken less than six months ago.

For processing time of less than two weeks, you will need to provide a confirmed/proposed itinerary or copy of your air tickets that reflects a destination outside of the U.S.
If the second parent or legal guardian is not available to sign, he/she will need to fill out a
Statement of Consent or Special Circumstances: Issuance of a Passport to a Minor under Age 16 (DS-3053). This form must be NORTARIZED and should be submitted along with DS-11. - $120.00 in check or money order payable to "U.S. Department of State". Please note that starting check will not be accepted.
Attach the following outside the sealed envelope:

Two Letters of Authorization signed for by one of the parents or legal guardians.
These are form letters authorizing our company to process your passport
request.

Processing Order Form. This will allow us to see the name of the applicant, along with the date of birth and date of departure.
- If you are also applying for visas(s) through us, all the requirements for visa(s) must be left outside the sealed envelope.

- Passport Card (applying along with Passport Book at the same time): $20.00

Passport Processing Order Form with credit card information. Application must be sent via FedEx First Overnight or
UPS Early AM Delivery, and must be marked "Signature Released" on package. Application that is not received at our office by 8:00am on the date reserved is subject to a charge of the full service fee. - Passport Card
